  2. **under “Forms” fill out the online initial intake form and submit.

  3. **under “Forms” please read “preparing for your first visit” before your appointment.

Follow-up and Initial visits

Cancellation policy: We do ask that you try to cancel or reschedule all appointments with 24 or more hours notice. No shows will be sent a bill for the full amount of the missed appointment. Same day cancellations will be auto-billed $55. Thank you for your understanding!
